1. Identify the region for which you are consulting (headache, back pain, lower or upper limb) .

2. Please select the service in your preferred language (French = initial exam) (English = initial exam). In the list, english appointments follow.

You will then receive an appointment confirmation email. Please complete the forms attached to this email so that the chiropractor has an overview of your condition and state of health.
